Forth Valley Carers Card
The Forth Valley Carers Card is a carer identification card that has been developed by local carers, Carers Centres, NHS Forth Valley and local authorities to ensure that carers are more easily identified by health and social care professionals and recognised and valued as equal partners in care.
The Forth Valley Carers Card will help to ensure that Health and Social Care professionals:
- Recognise carers
- Listen to the views of carers
- Provide carers with information to help them in their caring role
- Involve carers as equal partners in care
The Forth Valley Carers Card will not:
- Authorise Health and Social Care professionals to provide confidential information
- Provide discounts or privileged access to services
- Trigger a response from Health and Social Care services in the event of an emergency
Use of Personal Information
Falkirk & Clackmannanshire Carers Centre Ltd will hold information regarding you as a Carers Card holder on our system. We will do so in line with the Data Protection Act 2018.
